Four spanning the Region, that are delivered through a number of programs across the Geelong and Barwon South West region. and world renowned that that advances health and wellbeing for all.About the facilityPost-acute services at Barwon Health provide care to people who have been discharged from emergency, sub-acute and acute services and have been assessed as requiring short term, community based supports to assist them to recuperate in the community and avoid unnecessary representation to hospital.The Post Acute Clinician will contribute to the safe and timely discharge for people from public emergency departments, acute and sub-acute services, who have been assessed as requiring short term, community-based supports to assist them to recuperate in the community and avoid unnecessary re-presentation to hospital as per Health Independence Program guidelines. Desirable: • Post graduate health qualification health related field
