Applications are sought from suitably qualified Registered Nurses to work in the IPAC at Echuca Regional Health.
Key responsibilities include:
- Provide safe and comprehensive patient focused care coordination
- Provide rapid response and facilitate early discharge from public hospitals by purchasing brokered services
- Provide appropriate short term goal directed care
Our team requires someone who has:
- Previous care coordination experience
- Demonstrated problem solving and decision making skill
- High level interpersonal skills
