The Role:
Post-Acute Care (PAC) is part of the MBPH@Home programs. PAC is funded by Department of Health and Human Services and assists clients to recuperate at home following discharge from a public hospital.
The Post-Acute Care Coordinator is the primary contact for patients requiring support on discharge from MBPH. The PAC Coordinator assess needs, identify risks, coordinates care and facilitates appropriate supports for clients who are referred to the Post-Acute Care program.

To be successful in this role, you will have:
- Previous experience in a customer service related area
- Effective communication and interpersonal skills
- Proven ability to work effectively in a team environment and independently as required
- Strong customer service skills
- Strong attention to detail
- Strong keyboard and computer skills
- Ability to complete tasks within designated timeframes.
