Identitywa is one of Western Australia's leading disability support agencies. For over 45 years, we have been supporting people with a disability and their families. Since 2016 participated in a national study led by La Trobe University designed to further embed person centred active support and frontline Practice Leadership.
As Practice Lead Mentor you will report directly to the Executive Manager Services and will be responsible for:
· Leading and implementing Identitywa’s Person Centred Active Support Framework
· Coaching, mentoring and training key stakeholders to embed person-centred active support
· Retaining a focus on staff practice, monitor data and target actions to improve service quality
· Developing and maintaining resources and tools to assist participants and families with supported decision making
The successful applicant will have:
· Exceptional written and verbal communication
· Proven experience in supporting change and coordinating key projects
· A relevant tertiary qualification or equivalent work experience
· Demonstrated experience in person centred practice in a disability setting
· Previous experience in mentoring and coaching people in the workplace
· Ability to develop resources, content and reports tailored to the intended audience and consistent in key messaging
