Role Description
The primary function of the Progress Support Consultant (PSC) is to attend & facilitate multidisciplinary care meetings between participants, families, care teams, and community organizations on a quarterly basis. Assessing progress and development of participants engaged in NDIS support through the Together We Can International.
Reviewing systems, goals, progression and potential barriers that have arisen throughout the duration of support provided, updating Tailored Support Plans and liaising with care teams in a collaborative approach. Providing progress reports quarterly or as requested by the support team. Identifying new or continued support strategies to assist in meeting the goals of the participant that inline with the goals of the organization whilst providing coaching, mentoring & strategies to staff directly supporting those participants to work towards achieving greater outcomes and ensure quality, personalised individual support for all participants.
Non-Negotiable Requirements
- Bachelors Degree or Tertiary Education Equivelent in allied health
- Drivers licence & own reliable vehicle
- NDIS Clearance
- Working with Children Clearance (WWC)
- Excellent Communication skills
- Report writing skills & experience identifying barriers and implementing strategies
- Excellent admin skills & knowledge
Highly Desirable
- Experience working with people with a Disability
- Lived experience with Disability or working with vulnerable people
- Previous experience supporting young people
- Recruitment, Employment Service or Human Service experience
- First Aid, Mental Health, Youth work, Social work or Human Services qualifications or experience
