Hesse Rural Health is a key integrated rural health care provider located within the three local government areas of Surf Coast, Colac Otway and Golden Plains. The organisation provides a broad range of services which include Aged Residential including Dementia Specific Care, Sub-Acute Hospital, Urgent Care, Community Health, Health Promotion, District Nursing, Allied Health, Palliative Care, Home Care Packages and Planned Activity Groups Services operate from sites at Winchelsea, Beeac, Rokewood and Bannockburn. Hesse provides a community based, multi-disciplinary team approach to the identified health needs of the population and aims to promote the wellbeing of the rural community it serves.
The Quality Co-ordinator – Residential Aged Care will:
- Assist with the coordination of the quality improvement program, facilitate the delivery of an effective program and assist with the collection and submission of quality indicator data.
- Be the champion for the Platinum 6 electronic record system in Aged Care and lead the implementation of the electronic medication system.
- Support the coordination of efforts towards Residential Aged Care accreditation and support the coordination of accreditation requirements for all areas of the organisation.
- Formulate effective working relationships within the organisation and be a key resource to provide support and education to staff.
- Oversee the AN-ACC system , ensuring that care plans, assessments and documentation are comprehensive and current, in order to ensure consumer classifications are accurate and reflect care needs, and optimize income.
- Support the co-ordination for reviewing and undertake updates of Residential Aged Care policies and procedures.
- Utilise the My Aged Care website to manage AN-ACC, submit SIRS and update and retrieve relevant data.
Key Selection Criteria
- Demonstrate understanding of policy, legislation and professional standards governing Residential Aged Care service delivery.
- Understanding of AN-ACC funding processes desirable.
- Project Management experience desirable.
- Platinum 6 experience including electronic medication management desirable.
- High level of computer literacy, especially with Microsoft Word, Excel, PowerPoint, Visio and Project to produce professional documentation and presentations.
- Well developed organisational skills, with demonstrated capacity to successfully manage competing priorities, maintain attention to detail and meet deadlines.
- Ability to work as a team member as well as independently. Demonstrating self motivation, initiative, adaptability and professionalism.
- Competent interpersonal skills with the ability to convey information and communicate and engage and work collaboratively with diverse stakeholders. Proven ability to communicate, both verbally and in writing, in a clear, concise and logical and effective manner.
