Why Latrobe Community Health Service?
At Latrobe Community Health Service, we aim to prevent health problems through information and awareness, and to intervene sooner through the delivery of truly integrated healthcare.
Our origins are in Gippsland regional Victoria, but we have grown rapidly in recent years. Our services include general practice (GP); nursing; allied health; dental; counselling and psychology; gambling support; alcohol and drug treatment; migrant and refugee settlement services; social connection; carer support services and National Disability Insurance Scheme (NDIS) in partnership with the NDIA.
What you’ll be doing in this role?
As a RAS Assessment Officer, you will visit older community members in their home to assess their eligibility for entry level Australian Government-funded home care support, so they can remain living independently at home.
"Your staff member was very knowledgeable and able to answer my questions and concerns in an honest way, while also being respectful of my dad's view on things. I felt she really listened to my concerns and provided real solutions. Thank you!"
This sort of feedback is the reason people work in the Regional Assessment Service (RAS) team at Latrobe Community Health Service. You can leave work each day, knowing you have helped elderly, vulnerable and culturally-diverse clients to keep living independently in their own home.
Your work will include:
- Undertake assessments for people from vulnerable groups, including those from a culturally and linguistically diverse background (CALD) and an indigenous background
- Maintain the accuracy of client records in organisational client management systems and the My Aged Care (MAC) Assessor Portal
- Actively participate in and contribute to team meetings and quality improvement activities within the RAS team
What we’re looking for:
- Certificate IV in Aged Care, Disability, Social Services (desirable)
- A current Victorian Drivers Licence
- A current Working with Children Check (Employee)
- Ownership of a registered vehicle with minimum third party insurance
- Ability to travel across the service delivery area
What’s in it for you?
- Up to $15,900 tax free income through not-for-profit salary packaging per year
- An optional $2,650 tax free to spend towards meals and entertainment
- Access to discounted Private Health Insurance
- Training & Development (Online & Onsite)
- Career Progression Opportunities
