Job Summary
The Registered Nurse Transition Care Program (TCP) is responsible for specialising clinical services; supporting TCP Team and general operation of the transition care service, including recipient documentation and care planning and management; to ensure eligible clients’ initial and ongoing assessment, planning and management of care is undertaken within the TCP scope of practice.
Primary Duties and Responsibilities:
Specialised TCP Clinical Services
- Assess for pain and a plan implemented to keep the care recipient as free from pain as possible.
- Provide care and maintain tubes, including enteral feeding, naso- gastric and tracheostomy tubes etc.
- Establish, review and maintain urinary catheter care and/or stoma care program.
- Ensure complex wound management where applicable.
- Administer enema or insertion of suppositories as required.
- Suctioning of recipients’ airways and tracheostomy care.
- Ensure where required, oxygen therapy under ongoing supervision due to recipients’ care variable needs, including provision of oxygen and oxygen equipment at no cost to the recipient.
- Ensure appropriate medication management.
- Provide appropriate nursing services to recipients.
- Provide dementia support where required.
- Take appropriate action to prevent falls among care recipients.
- If required, facilitate or provide on-call access to specialist nursing services.
Therapy Service Support
- When required facilitate therapeutic care to be delivered through TCP, which includes low intensity therapy such as physiotherapy, occupational therapy, podiatry, speech pathology, counselling and social work to maintain and improve physical and cognitive functioning and to facilitate improved capacity in activities of daily living.
Facilitate that day-to-day diabetes education and management forms part of ‘dietetics’ and is undertaken by qualified diabetes educator who overseas and manages diabetes therapy clinically appropriate, according to care recipients’ need and care plans.
The successful candidate must have the following essential certifications.
- Registered Nurse with valid AHPRA Registration Division 1
- Bachelor of Nursing
- WA Valid Driver’s license
- Police Clearance no more than 3 months old
- Valid Working with children’s check
- First Aid or willing to obtain.
- COVID-19 Vaccination Certificate
Skills, Abilities and Knowledge:
- Experience working with First Nations people
- Excellent communication and interpersonal skills, with the ability to build trust with community.
- Demonstrated nursing experience with Transition Care Program.
- Ability to organise and implement community based activates within allocated budget.
- Ability to work in a multi-disciplinary team in health setting.
- Demonstrated understanding of the principles of person-centered care.
- Good record management, data collection and reporting capability.
- Ability to work autonomously and be a self-starter, with a high degree of initiative and problem-solving skills across multiple sectors.
