About the role
This role aims to address the growing number of clients with complex multi-morbid health conditions and social care needs, where factors such as frailty, impaired cognition and polypharmacy add to the difficulty of planning and navigating care requirements congruent with the client’s expressed wishes.
Under the mentorship of our Dementia Nurse Consultant, you will be supported to undertake impeccable systematic clinical assessments incorporating the application of critical thinking across the bio- psychosocial paradigm. Coordinating case conferencing and care planning would also be considered a key function of the role, where case management is not in place.
Skills & Experience
- AHPRA recognised Registered Nurse qualification
- Post graduate qualifications and/or extensive experience in aged care nursing
