The Role
This is your opportunity to work Monday - Friday standard business hours giving you a work-life balance. Free onsite parking and the opportunity to WFH.
You will be responsible for integrating and coordinating residents care and support requirements.
This is a leadership role where you will interact and work alongside internal and external stakeholders.
Responsibilities:
- Support the entry process for residents moving into the home
- Identify eligibility for externally funded care
- Access Government funding for eligible residents
- Determine eligibility for other external funding
- Liaising with a large Multidisciplinary team
Qualifications:
- Current full AHPRA registration as a Registered Nurse (RN) with no conditions/restrictions
- Previous experience in Aged Care or Home Care Package management.
- Excellent communication and interpersonal skills, with the ability to connect with residents, families, and colleagues.
- Empathy, patience, and a genuine desire to help
- Ability to work collaboratively in a team-based environment.
