Job description

Work for one of the largest and most trusted not-for-profit providers of Aged Care in Australia who are transitioning to Household Living - an innovative approach to care - which empowers residents to live each day, their way, just as they would in any family home.

The facility has 4 separate households, each with between 15-24 residents, including a Memory Support Unit for people living with dementia. The Leisure and Lifestyle Coordinators ensure that the community is thriving and doing things that are meaningful and enjoyable. The best part is you get to know each resident and become a fundamental part of their life. You support not only their clinical care, but you play an important role in making sure the care provided meets their individual goals.

About the position:

  • The Household Model of care empowers the residential care teams with freedom to support residents in ways that help them maintain a sense of meaning and purpose in their day
  • You will be a clinical leader in the service and you will develop, deliver and evaluate resident care plans to meet the individual needs of our residents
  • You'll need good clinical assessment skills and will take the lead in providing clinical services
  • You'll liaise with medical and allied health professionals and resident's family members to advocate for clients' quality of life
  • Supervise and support Personal Care Workers
  • You need to practice in a holistic way, using your critical decision-making skills to lead the team.

About You:

  • Care and commitment for our Aged Care clients
  • A current and unrestricted AHPRA Registration
  • Experience as an Aged CareRegistered Nurse is an advantage but we encourage New Graduates to apply
  • Demonstrate judgement and confidence in your Nursing ability
  • Commitment to ongoing professional development
