About the Care Coordinator (Registered Nurse)
As a Registered Nurse, you'll utilise your clinical, change management and communication skills in a new program to support clients living with chronic/complex conditions to improve their self-management and help them stay safe and well at home. You'll be responsible for individually tailored client care plans and take a lead in building referral pathways and social prescribing networks.
You'll also provide supervision and support to non-clinical Care Navigators ensuring the client receives the "right care, at the right time".
This is a part-time (0.6FTE) fixed term opportunity reporting to the Team leader, Integrated Care.
Your passion and commitment will see you:
- Join a new community based integrated care program as part of the enthusiastic and fantastic Integrated Care team.
Provide assessment, care planning, referral, coordination and social prescribing for client caseload.
Build referral and social prescribing networks through collaboration and change leadership.
Supervise and support non-clinical Care Navigators and collaborating to improve client outcomes & experience.
To succeed in this role you will have:
- Relevant Qualifications and current (unrestricted) AHPRA Registration as a Registered Nurse.
- Minimum 5 years clinical experience as a Registered Nurse.
- Experience working with people living with complex and chronic conditions.
- Excellent interpersonal skills, flexibility and change leadership skills.
Ability to tailor individual care plans and collaborate with non-clinical staff to implement.
As a YourCH employee in this role you must have:
- A valid Victorian Working with Children Check (employee only)
- A Satisfactory Police Check (paid for by YourCH)
- Victorian Drivers License
