About the role
- Provide service coordination and case management for patients with inflammatory bowel disease, hepatitis, and decompensated liver disease.
- Follow up with patient's post-endoscopy.
- Offer support, treatment options, and education for patients with GI health conditions and related clinical alcohol and drug issues.
- Develop networks with General Practitioners and allied health providers in the South West region.
- Engage in a shared care model with specialist clinics and facilitate early primary care management.
- Deliver specialist outreach services to remote areas via telehealth.
- Liaise with health and welfare agencies, including mental health, alcohol and other drugs, and Aboriginal services.

- Registered Nurse Grade 2, minimum of 5 years clinical experience

- Serve as a clinical nursing/midwifery resource and advisor to ward staff and South West healthcare service providers.
- Participate in team planning and development activities within the clinical specialty area.
- Undertake projects and research to address specified clinical nursing needs.
- Adhere to professional standards, codes, and behaviours as legislated by the Nursing Midwifery Board of Australia (NMBA), other relevant professional bodies, and SWH Policy, Standards, and Practice Guidelines.
