About us:
Hospital in the Home (HITH) is a medically governed service that provides care in a patient's permanent or temporary residence for conditions requiring clinical governance, monitoring and/or input that would otherwise require treatment in a traditional inpatient hospital bed.
Post - Acute Care Services (PACS) referrals are post-surgical patients that require negative pressure wound therapy or have complex wound care needs.
About the role:
- The incumbents will work across all clinical units within The HITH and Post-acute care services.
- Weekend shifts (seven (7) day roster) and on-call (no call-out) is required for this role.
- The position is required to work within HITH and Post-acute care programs. The shift patterns for this role include early to staggered starting hours to finish at 1900hrs. On-call shifts (not called out, phone calls only) are also a shared responsibility of this role.
- Provision of nursing care that meets all relevant competency and professional practice standards relevant to the position.
- Deliver a high standard of patient care including assessment, planning, implementation and evaluation by using clinical nursing/midwifery expertise.
- Respond appropriately to sudden alterations in The health status of the patient using comprehensive nursing assessment and intervention skills.
About you:
To be successful in this role you must have:
- Current nursing registration through the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum of three years nursing experience.
- Demonstrate an understanding of the additional considerations for working in a patient's home environment, including measures to keep patients and themselves safe.

Total remuneration value of up to $9,798 p.m. is comprised of:
- A salary between $40.51 - $51.97 p.h.
- 12.75% employer superannuation contribution
