What you'll be doing
- The purpose of the inTouch Registered nurse it to provide holistic nursing care to patients who are at risk of hospital due to complex care needs due to health conditions such as, chronic disease, frailty due to aging-associated decline and COVID.
- The objective of the position is to maximise the health outcomes for patients through conducting health assessments, providing care coordination between health care provides, assisting patients to access services and communicate health information to improve a patient’s health literacy so they can better manage their condition.
- Intouch is a community-based service within a multidisciplinary team that provides care mainly as a telehealth service.

- Registration: Current Registered Nurse Division 1 National Registration APRHA
Selection Criteria
- Previous relevant clinical experience in providing complex care as part of a multidisciplinary team
- Demonstrated effective interpersonal skills and ability to work effectively with a range of health professionals/service providers
- Demonstrated experience in care coordination and care navigation, including effective development of care plans and identification of appropriate referral pathways.
- Demonstrated experience in facilitating education and self-management support for clients with complex care needs.
- Experience and working knowledge of Information Technology systems currently used in providing patient care in WSLHD such as CERNER, iPM and CHIME. Experience and knowledge working Microsoft applications such as Word, Excel and Outlook
