About the role

The Rehab/GEM @Home team is looking for positive and practical nurse(s) to join the team in a full time or part time permanent roles as a 'road nurse'.
GEM@Home is run concurrently with Rehab@Home and are fortunate enough to be expanding to create a large at home care service for our hospital patients. Together, both programs provide multidisciplinary care to adults, including Medical (Geriatricians and Rehab Physicians), Nursing and Allied Health bed-substitution and hospital/ED diversion beds in the community, with an expansion to 43 beds across the broad and diverse Eastern Health catchment.
The growing Nursing team will report to the Nurse Coordinator/Team Leader. We also have an Allied Health Team Leader, the full-suite of Allied Health staff including Pharmacy, and multiple Medical practitioners who all work collaboratively and innovatively to promote patients and their families to achieve their recovery and/or rehab goals.
The team culture is warm, engaged, and passionate about patient-centred care and we are keen to find a like-minded Nurse to join us.
About you
To be successful in this role you will possess
- Current Nursing registration with AHPRA
- A strong commitment to high quality care and thorough knowledge of the healthcare standards which underpin this
- An ability to quickly build rapport and trust with varied patient cohorts and their families, as well as with colleagues
- A proven record working as part of a diverse, multidisciplinary team, as well as the capacity to work independently
- A diligent and reliable work ethic with an ability to manage competing demands and display time management skills
- Highly developed interpersonal, written and verbal communication skills
- Ability to work in an at-times fast paced and rapidly evolving program
- Sound IT skills including use of MS Office, mobile phones, MePACS, GPS in cars.
- Ability to participate in a 7-day work week including rotational rostering across weekends and public holidays.
- Current and appropriate car licence
