An exciting opportunity has opened up with the Health Contact Centre in the Department of Health! We are seeking an experienced and proactive Registered Nurse (Self-Management of Chronic Disease) to join our team. In this role, you will be responsible and accountable for delivering excellent patient focused, evidence-based nursing care in a specified field of practice (chronic disease management) and you will also have the flexibility to work from home up to 50% of your hours.
The Registered Nurse is a member of the multidisciplinary health care team and uses contemporary standards and evidence to underpin practice. The Registered Nurse, Self-Management of Chronic Disease (SMoCC) reports to the Clinical Nurse SMoCC.
