Form of workAn exciting opportunity has opened up with the Health Contact Centre in the Department of Health! We are seeking an experienced and proactive Registered Nurse (Self-Management of Chronic Disease) to join our team. In this role, you will be responsible and accountable for delivering excellent patient focused, evidence-based nursing care in a specified field of practice (chronic disease management) and you will also have the flexibility to work from home up to 50% of your hours.
The Registered Nurse is a member of the multidisciplinary health care team and uses contemporary standards and evidence to underpin practice. The Registered Nurse, Self-Management of Chronic Disease (SMoCC) reports to the Clinical Nurse SMoCC.

The HCC operates 24 hours a day, seven days a week delivering confidential telephone assessment and information services to the people of Queensland. The HCC strives to educate and empower people to make informed choices about their health and health care, promoting better health through the delivery of evidence-based clinical practice.
