Northern NSW Local Health District are looking for Palliative CareRegistered Nurses to work with members of the Palliative Care nursing team (including CNCs and CNS’) across the inpatient and community settings in Grafton and Lismore. The aim of the Palliative Care Nursing service is to facilitate a coordinated, high quality Palliative Care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care.The service supports GPs, Community Nurses and the Residential Aged Care In-Reach Team in the assessment, care and management of patients with life limiting illnesses and takes a multidisciplinary approach to care with the end goal of improving quality of life for those patients and their carers.
If you have/are:
- Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1) and experience in working with patients requiring supportive and end of life care.
- Recent knowledge and clinical experience in working with patients requiring supportive palliative and end of life care including experience working in inpatient and community care settings
- Ability to use problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care
- Effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others
