Part-Time, 2 Days/Week

Are you a dedicated and compassionate Registered Nurse with a passion for community service? Charlestown Caring Group is seeking a skilled and motivated individual to join our team as a Community Service Registered Nurse on a part-time basis, working two days a week. This is a unique opportunity to make a meaningful impact on the lives of individuals in our community while taking a leadership role in the Care Coordination Team.

Responsibilities

As the Registered Nurse Team Leader, you will play a pivotal role in the supervision and management of our Care Coordination Team a highly professional team with extensive experience in the aged care and disability sectors. Key responsibilities include:

Team Supervision and Management

  • Oversee and lead the Care Coordination Team in delivering high-quality community nursing services.
  • Provide guidance, support, and supervision to team members, fostering a collaborative and positive work environment.

Incident Review and Management

  • Conduct thorough reviews of incidents, working closely with team members to ensure comprehensive understanding and resolution.
  • Collaborate with the Support Services Team Leader and People and Culture Business Partner to address and manage incidents related to clinical areas, such as diabetes management, skin care, and medication management.

Education and Training

  • Develop and implement educational programs focused on skill offering required for best practice community support health services focusing on diabetes management, skin care, and medication management.
  • Ensure staff are well-educated and competent in recognising and reporting incidents, maintaining high standards of care.

Collaboration and Communication

  • Foster effective communication within the team and across departments to ensure seamless coordination of care.
  • Collaborate with healthcare professionals and external partners to enhance the overall quality of care provided to our community.

Eligibility

  • Valid and current Registered Nurse license.
  • Proven experience in community nursing and leadership roles.
  • Strong knowledge and expertise in clinical areas, specifically diabetes. management, skin care, and medication management.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a multidisciplinary team.
  • Current A.H.P.R.A Registration.
  • Working with Children Check.
  • NDIS Worker Screening check.
  • NDIS Worker Orientation Module ' Quality, Safety & You'.
  • First Aid Certificate & Level 2 (including CPR).
  • Federal Police Criminal Record Check.
  • Evidence of Immunisation history including C19 digital certificate and current flu vaccine.
  • Reliable, comprehensively insured vehicle.
  • Smart Phone with active voicemail & access to the internet.
  • Manual Handling Competency. 
  • Current and valid Driver’s Licence and insured, reliable motor vehicle.

Desirable

  • Aged Care, Community or Disability Experience within the community setting
  • 12 months post grad experience is preferred but not essential
