About the Role
The Residential InReach (RIR) program works collaboratively with residents, families, Residential Facilities and General Practitioners, providing acute assessment and intervention to receive traditionally based hospital care in setting of choice reducing the need to come into hospital.
The primary aim of the position is to provide specialist assessment and implement clinical management of residents in Residential Aged Care Facilities (RACF’s) in order to prevent avoidable presentations to the Emergency Department (ED)
Duties
- Provide comprehensive clinical assessments, care planning and interventions to residents with complex care needs across Wellington Shire Aged care facilities to prevent avoidable hospital presentations and admissions.
- Provide leadership and advocacy to patients across the care continuum, facilitating prompt and effective care, supporting patients identified goals in setting of choice.
- Strengthening care networks and referral pathways with General Practitioners, Ambulance Victoria & Victorian Virtual Emergency (VVED) will be an critical focus in this role in supporting better health outcomes for the older person in aged care.
- Reducing unnecessary presentations, developing strategies to reduce patient time spent in ED and general advocacy of the older person, and support of ED staff in the caring of older people.
About You
- Experienced Registered Nurse with strong clinical background, highly developed assessment, clinical management, problem solving & prioritisation skills
- Energic with a passion to achieve better health outcomes in Aged Care
- Excellent communication skills including an ability to develop rapport with people quickly
