We are seeking a motivated and enthusiastic Registered Nurse or Allied Health clinician (Occupational Therapy, Social Work and Physiotherapy) to become part of the Western Health at Home Community Care Coordination team. This is a full-time permanent role across the Community Care Coordination teams. This is a really exciting time to be working in Care Coordination at Western Health, where we are focused on innovation, improving and optimising our management of patients in their homes.The Senior Community Care Coordinator is essential to the Western Health at Home teams in providing clients and carers’ with care coordination and management of their chronic illness and navigation of the Health and Primary care system. The Senior Care Coordinator is responsible for comprehensive patient assessments, providing self-management support and coordinating patient healthcare needs across the health care continuum.The aim of the service is to:
- Effectively coordinate the care of people with Chronic and complex conditions to self- manage their health needs
- Provide an alternative to hospital presentation and admissions
- Provide support for patients to remain at home following a change in health or functional ability
- The Senior Clinician/Nurse is a clinical lead across the Community Care Coordination teams
- Have minimum 7 years’ experience in public health
- Have a passion for and experience in providing high quality patient centred care
- Have relevant acute, community and care coordination experience
- Have excellent interpersonal communication skills
