WHAT THE ROLE LOOKS LIKE The role of The Senior

Case Manager is to provide case management support to tenants in the Towards Independence Program Top End. The role works alongside tenants to develop and implement case plans to achieve self- determined goals.

This role will be responsible in maintaining support for the reporting requirements for The Salvation Army, provide case management and coordination support. The Senior

Case Manager will be required to assume the role of the Supported Housing Program Manager in their absence. The Senior Case Manager is to participate in the professional operations and delivery of supported accommodation and psychosocial support for tenants some with low to mild mental health and episodes of intensive mental health needs.

Implement and facilitate a variety of programs including, Life Skills Program, Positive Lifestyle Program and Mothers Group. ADDITIONAL ROLE RESPONSIBILITIES Tenants are supported and encouraged to achieve own goals within established case plans including assisting tenants with appointments and advocating for tenants with other services providers. Undertake intake and assessment with tenants to develop a shared understanding of their situation and related problems and strengths.

Accurate, up to date client records and data is maintained for all tenants in accordance to TSA standards and procedures within TSA’s identified electronic database. Ensuring appropriate preventative action is taken in relation to potential high-risk situations, as per policy and procedure. Advocate on behalf of tenants with various government and non-government agencies, both verbally and in writing.

Progress outcomes of individual case plans and regularly monitor and review and take action to ensure case plan continues to meet tenants needs and goals. WHAT YOU WILL BRING Degree qualification preferred, in Social work, or Welfare, Community Development and with related field work experience and demonstrated high level knowledge in the sector and/or skills attained through previous appointments, service and study/development. Or a minimum Certificate IV in Community Services in Social

Work, Welfare, Community Development and demonstrated related high-level work experience of sector knowledge and skills attained through similar appointments, service, and study/development A National Police Record check is required prior to commencement. A NT Working with Children Ochre Card check is required prior to commencement.

A current First Aid certificate or willingness to obtain. A current NT driver’s license.

Be able to provide proof of eligibility to work in Australia. A Certificate of Full Vaccination status for Covid-19.
