About the role
- The purpose of this role is to apply high-level Social Work skills and knowledge in the delivery of psychosocial care to consumers and their families/carers of the Community Palliative Care service.

- Clinical Expertise: demonstrated high-level professional skills, knowledge andexpertise in Social Work practice to deliver quality clinical care to consumers receiving end of life care and their families/carers
- Communication/Team participation - Communication/Team participation: demonstrated ability to build and maintain effective consumer and professional relationships in a multidisciplinary team environment, applying high level written and oral communication skills, adapting approaches to meet individual needs in delivering psychosocial care to consumers and their families/carers.
- Leadership and Management - demonstrated ability to initiate and lead local service improvement initiatives through continuous quality improvement; demonstrated ability to provide professional leadership within the multidisciplinary team and Social Work discipline, acting as a reference point on both complex and routine clinical practice matters for HP3 clinicians.
