Form of workWe have an exciting opportunity for a passionate and motivated individual to join Civic Disability Services in the position of Senior Support Worker.
ABOUT THE OPPORTUNITY
Reporting to the Practice Leader, the role provides leadership in ensuring the team focus on delivery of Person-Centred Active Support to our clients. The Senior Support Worker has a well-developed understanding of our practices and processes. This role is located in South West Sydney.
On some days you’ll be needed on the ground, providing hands-on person-centred active support to help our clients reach their potential. On other days you’ll be liaising with service coordinators, health professionals, the broader community, as well as your internal stakeholders to ensure we are providing the right support at the right moment to create moments that matter.
Admin skills, attention to detail and team leadership are a must. Also, the drive to be involved in human centred service design, because as the service grows and changes, we need you to be flexible and ready to be creative.
DUTIES & RESPONSIBILITIES
- Lead the team to deliver Person Centred Active Support according to the Practice Framework in all aspects of Client care.
- Provide operational administrative support which can include roster support, performing site inductions, coordination of team meetings/case conferences etc.
- Advocate for Clients, providing the right amount of liaison between families, guardians, community groups, healthcare professionals, teachers and other services in the individual’s life.
- Ensure Support Workers receive thorough and appropriate induction enabling successful integration to the Client’s life.
- Raise learning needs/ideas with the Practice Leader to support the team’s development and improve client outcomes.
- Assist the Practice Leader to manage the budgets, administrative and financial systems and records of the program and monitor their use in accordance with the organisation’s requirements.
ABOUT YOU
Your experience in the heath, disability or aged care fields has provided you with the necessary experience to help support those around you. Your knowledge and understanding of the disability sector is strong. You understand the environment and are proactive about these rapidly evolving conditions.
We are looking for applicants who exhibit the following:
- Certificate III or IV in community services, disability, aged care or related field. You may also be studying towards a qualification in social work, nursing, allied health or similar.
- Experience as a Support Worker providing person-centred care. An understanding of how direct client experiences are coordinated and delivered.
- Ability to build strong working relationships with different stakeholders such as clients, families, colleagues, leaders and allied health professionals.
- Creativity to be able to work clients and team members to meet client goals.
- A passion for creating incredible client experiences that allows people to realise their potential.
