Focas Shoalhaven Inc is a NDIS registered provider supporting adults with disability with social, recreational and daily living activities and support coordination. We deliver 1:1 supports and focus on ensuring our participants lead meaningful and independent lives. We have been operating in the Shoalhaven for over 25 years and have an excellent reputation for providing individually tailored support. We are a small very cohesive team supporting 75 participants predominantly in the Shoalhaven. We pride ourselves on providing very personalised service and matching support staff to participant needs and preferences.
Qualifications & experience
- Tertiary qualifications in relevant fields
- Minimum 1 year experience in similar role with exposure to the NDIS
- Experience in using end to end care management systems
- Rostering experience
Tasks & responsibilities
- Coordinate individually tailored support services for NDIS participants according to individual budgets.
- Create service agreements and update as required
- Monitor NDIS participant budgets
- Intake and onboard new NDIS participants
- Liaise with external parties
- Ensure compliance with NDIS regulations and organisational policies
- Source and match staff appropriate to participant needs and preferences
- Manage participant data in the customer relationship data base
- Assist with rostering and administration tasks when required
- Be on call on a rotational basis
