Job description

  • Contribute to the implementation of service improvements along the client journey
  • Contract to 30 June 2025, 1.0 FTE (Part-time negotiable for the right candidate)
  • Salary range $100,479 - $104,095 (pro rata for part-time) + superannuation + salary packaging benefits
  • Work in a collaborative team across Melbourne’s West + flexible hybrid working model

The Opportunity

An exciting opportunity exists for a highly motivated Service Integration Partner to make a real impact to client health care experiences and outcomes.  This role will be a key player in  implementing identified service model improvements alongside operational teams to achieve IPC Health’s Service Model Transformation Strategic objectives.  These objectives include greater  navigation and co-ordination of services, interdisciplinary ways of working, continuous learning and technological advancements.

As a member of the Service Model Transformation Team, this role will collaborate with a range of stakeholders to build organisational capability in the integration of service innovations across programs and provide hands on project delivery.  This position is ideal for someone who thrives in a fast-paced environment and enjoys taking a people focus approach in fostering a culture of service excellence.

The ideal candidate will have experience leading teams through change initiatives, have a clinical, management or improvement background in the health, social services or NFP sectors, possess highly developed communication skills, maintain excellent attention to detail and be someone who enjoys working autonomously as well as in a team environment.

  • Developing implementation and transition plans that successfully guide the integration of service improvements along the client journey which deliver the following outcomes:

  • Improved workflow efficiencies
  • Co-ordinating, delivering and monitoring service model improvement initiatives in partnership with operational managers and their teams.
  • Fostering the enabling conditions that support service re-design in partnership with key stakeholders for sustained organizational growth and development.
  • Taking a people focused approach as workforce participation will play an integral part to enable changes.
  • Organizing multiple priorities and delivering to tight timelines.
  • Regularly reporting on achievements, challenges and limitations.
  • Contributing to Scrum, Kanban and Agile ceremonies.
  • Contributing to a Learning Health System.
  • Driving the development of implementation strategies which deliver desired outcomes such as:

To succeed you will need

  • Qualifications in health, management / leadership, improvement or related discipline and/or experience implementing process improvements within the health, social services or not for profit sectors.
  • Experience in the development and delivery of implementation and transition plans.
  • Strong problem-solving skills with a growth and beginner’s mindset.
  • Ability to influence and collaborate with a wide variety of stakeholders to integrate the adoption of innovative solutions that improve client experience, outcomes and processes.
  • To foster a culture of improvement through workforce capability building.
  • A high degree of emotional intelligence.
  • Attention to detail.
  • Sound report writing and presentation skills.
  • Effective communication, engagement and negotiation skills.
  • Change management experience within complex systems.
  • A current and valid Victorian Driver’s License

This position is full time, fixed term to 30 June 2025 and classified as Grade 5 (HS5) and (above award) depending on skills and experience. Under the HEALTH AND ALLIED SERVICES, MANAGERS AND ADMINISTRATIVE WORKERS (VICTORIAN STAND-ALONE COMMUNITY HEALTH SERVICES (MULTI EMPLOYER) ENTERPRISE AGREEMENT 2022 – 2026
