BACKGROUND
Since 1987, ACES has been providing programs and services to Aboriginal Elders. ACES was incorporated under the Associations Incorporation Act (1981) and have a Board of Management made up of elected community members.
In the Australian context, ACES is unique in being both Aboriginal community-controlled organisation and specializing in provision of both planned activity group and residential aged care services. At ACES culture is recognised as central to individual and community health. Further the integration of a range of programs on the site ensures continuity in the assessment and delivery of appropriate care to Aboriginal elders in a culturally appropriate environment as they age..

Amidst the hustle and bustle of East Brunswick, the Caring Place nestles in the serenity of lush, well maintained, bush-like gardens and the Merri Creek. The setting provides Residents with a friendly, relaxed culturally relevant home-like environment which is important to their health and well-being.
ROLE PURPOSE
The Social and Emotional Wellbeing (SEWB) Manager will be responsible for rolling-out ACES’ Social and Emotional Wellbeing Services and programs. Working with our residential and outreach care programs you will provide practice leadership and direction to ACES staff in the delivery of Social and Emotional Wellbeing programs to meet the needs of our Elders in residential care and in the community. You will work closely with our health and clinical care partners to ensure an integrated and individualized service response. The SEWB Manager will also be responsible for the development of a Cultural Safety Plan for the Aged Care Sector and develop a Model of A&TSI Aged Care. In discussion with the Executive Manager SPR and CEO of ACES, Other Special Projects addressing the growth and development of ACES and the aged care sector will also be part of the SEWB Manager’s responsibility.
This is an exciting new role within ACES and will involve the following Key Activities:
- Roll-out and manage the SEWB services program for Elders.
- Implement the ACES SEWB Plan, including ACES residential services, outreach services and support, community engagement and cultural activities.
- Assess the needs of Elders living at home and prepare plans to meet their needs.
- Manage the in-home service responses for Elders where no other case managers are already involved.
- Liaise and work with other ACCOs and mainstream services to ensure Elders support needs are being met.
- Provide advice to elders and their families about their SEWB needs.
- Work with other ACES programs to provide services to elders.
- Maintain excellent record-keeping and keep ACES database up to date.
- Excellent written and verbal communication and time management. skills and the ability to manage multiple complex demands and meet timelines.
- Work positively and effectively in small project groups within a larger team.
- Attend meetings on behalf of ACES as required.
- Development of a Cultural Safety Plan for the Aged Care Sector and develop a Model of A&TSI Aged Care. About you:
We are looking for someone who is passionate about providing Social and Emotional Wellbeing Services and programs to Aboriginal communities. Our ideal candidate will have some experience in a clinical environment, with a background in social work, psychology or mental health counselling.
