Job description

  • Utilise your leadership skills and understanding of Social Prescribing to join the forefront of major systemic change in Mental Health and Wellbeing service delivery
  • Collaborate with a multi-disciplinary team to coordinate the role-out of an exciting new program
  • Full-time, SCHADS Level 5 ($47.50-$49.65/hr +super + NFP salary packaging), contract till end June 2025

About the Service
The Royal Commission into Victoria’s Mental Health System has challenged us to think beyond incremental improvement to deliver a Mental Health system that helps all Victorians live their best lives. In a region characterised by fragmentation, complexity, and access barriers, implementing the Geelong - Queenscliff Mental Health and Wellbeing Local (GGQ Local) is a key step in realising this goal.

The GGQ Local is delivered by a consortium of equal partners: Barwon Health, Watharoung Aboriginal Cooperative Ltd, Wellways Australia and ermha365. 

The GGQ Local is underpinned by positivity, hope and inclusion. No matter whether a consumer, carer or provider enters or seeks assistance via digital platforms, contact centre, or physical locations, the first engagement will always start with a simple question: “Welcome, how can we help?” Drawing on established services, networks, and pathways, the GGQ Local provides seamless integration between local, area and Wellbeing services for the benefit of consumers, carers, and providers. This is supported through standardised assessment tools, treatment and support processes, care plans and care coordination, colocation of services and regional navigational pathways.

The three core elements of our service are people, purpose, and place and we are seeking Mental Health professionals ready to be part of a central and accessible service that understands the needs of their community.

About You
Passionate about empowering individuals to enhance their Mental Health and well-being through community connections and non-clinical interventions, you are a strong advocate for access and opportunity for those in the community.
Additional attributes we are looking for include:

  • Have a strong understanding of the principles of Social Prescribing as well as a core understanding of a variety of models of practice which are complementary to supporting people with their Mental Health and Wellbeing
  • Knowledgeable in accessing a range of relevant community resources, particularly in the areas of health, drugs and alcohol, legal, income support, employment and recreation
  • Experience in leadership, with effective interpersonal and communication skills, including the ability to contribute positively within a team, influence and negotiate with others
  • Proven customer service in developing & delivering solutions to consumers, carers and families
  • Relevant degree with relevant experience or Associate Diploma with substantial experience or less formal qualifications with specialized skills sufficient to perform at this level

We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes, including (but not limited to) wearing appropriate PPE and regular hygiene and cleanliness practices. 

About the Role
As the Social Prescribing Implementation Lead, you'll be at the forefront of implementing the Social Prescribing program. Your primary responsibility will be to coach, mentor, and supervise the Social Prescribing staff, ensuring they effectively connect consumers with pertinent community-based, non-clinical services. Your guidance will empower them with the necessary skills and knowledge to engage positively with consumers, carers, and their families, thereby fostering meaningful community connections and holistic well-being.

The role entails:

  • Provide leadership, coaching and mentoring of staff to ensure they are competent in the delivery of Social Prescribing
  • Develop systems to capture and report on relevant data and KPIs
  • Actively plan budget expenditure of the program to ensure performance targets are met
  • Act as an advocate and mediator with other relevant services and professionals regarding the needs of consumers, carers and their families 
  • Facilitate constructive links with appropriate community, clinical and non-clinical services

This is a full-time role, predominately Monday to Friday. There will be the expectation that some weekend work may be required to build social connections and awareness of the GGQ Local through participation in community events.
This is a contract position, ending June 2025.

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver’s licence.
