Key Responsibilities

  • Advanced level, autonomous clinical judgement in advising treating teams regarding complex Consumer Transitioning
  • Provide expert guidance, leadership and advice in relation to processes, community networks and services, to manage complex transition consumers and discharge planning coordination
  • Develop and actively contribute to building the service capacity to address the complex service needs of consumers by providing clinical and strategic direction to clinicians and teams
  • Lead the coordination and development of inter-professional competence for all clinicians involved in the area of complex transition patients to improve consumer outcomes
  • Facilitate clinical and systematic escalations within a patient safety and clinical governance framework and ensure timely and effective solutions are addressed

Qualifications

  • Bachelor of Social Work 
  • Eligibility for membership to the Australian Association of Social Workers
  • A graduate certificate or greater within a relevant specialty area is highly desirable
  • Eligibility to become an Authorised Mental Health practitioner under the Mental Health Act 2016 would be well regarded
  • Drivers Licence  
  • Blue Card (GCH will cover cost)
  • Evidence of vaccination against Measles, Mumps, Rubella, Varicella, Pertussis, Hepatitis B
  • Tuberculosis screening may be required
  • Declaration of serious discipline history is required
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Temporary full time (2 positions, up to 30.06.2025 with possibility of extension)
  • Base salary ranging from $5395.60 - $5630.00 per fortnight

About us

The new Secure Mental Health Rehabilitation Unit (SMHRU) will open on the Gold Coast in February 2025. The 40-bed SMHRU will provide world class, compassionate, person centred Mental Healthcare that fosters recovery of consumers, families and carers for those consumers with complex Mental Health and addiction issues. Embodying Gold Coast Health philosophy of Always Care, the philosophy of the SMHRU will be to provide a therapeutic environment which conveys hope, optimism and a belief in recovery to empower an individual to live a meaningful and connected life not defined by their illness.
