Position Overview:
As a Social Worker, you will play a pivotal role in supporting veterans in accessing and navigating the healthcare system. You will work closely with veterans, their families, and interdisciplinary healthcare teams to develop and implement personalized care plans that address their individual needs and goals.
Your expertise in social work will be instrumental in providing advocacy, counselling, and support services to veterans, empowering them to achieve optimal health outcomes and improve their overall quality of life.
Key Responsibilities:
- Conduct comprehensive assessments of veterans' psychosocial needs, strengths, and challenges.
- Collaborate with interdisciplinary teams to develop individualized care plans that address veterans' physical, mental, and social well-being.
- Provide counselling, crisis intervention, and supportive services to veterans and their families.
- Advocate for veterans' rights and access to healthcare services within the VA system and the community.
- Coordinate referrals to external resources and support networks, including mental health services, housing assistance, employment resources, and financial assistance programs.
- Monitor and evaluate veterans' progress in achieving their goals, making adjustments to care plans as needed.
- Maintain accurate and up-to-date documentation of all interactions and interventions in accordance with regulatory requirements and organizational policies.
Qualifications:
- Bachelor's or Master's degree in Social Work from an accredited institution.
- Have a current/active membership with the Australian Association of Social Workers (AASW).
- Minimum of 2 years of experience in social work, preferably in a healthcare or veteran care setting.
- Knowledge of veteran-specific issues, including PTSD, military sexual trauma, traumatic brain injury, and substance abuse.
- Strong communication, interpersonal, and advocacy skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Commitment to upholding ethical standards and maintaining confidentiality.
- Experience with electronic health records systems preferred.
