A partner Aged Care home is seeking a Social Worker to support their residents and families as part of Care Guidance's Wellbeing Program. The Social Worker will provide comprehensive psychosocial assessments, counselling support, case management and individualised therapeutic interventions to better support the mental health and wellbeing of older adults residing in residential Aged Care facilities.
This is a casual role with a minimum of one day a week based in the Launceston area.
About You
Skills and experience:
- Social work experience working within the health, mental health, palliative care and/or Aged Care sector
- Interested in supporting older adults to live with dignity and to improve their mental health and wellbeing
- Confident practicing as the sole Social Worker within an Aged Care facility
- Sound clinical knowledge and skills and the ability to manage complex psychosocial issues
- Previous experience in providing 1:1 counselling support
- Excellent interpersonal skills and ability to proactively work with multiple stakeholders to build rapport and trust, whilst effectively articulating the role of social work; and
- Genuine interest and passion in achieving positive wellbeing outcomes for Aged Care residents and their families.
- Servicing social work referrals from all wards
- Potential of leading discharge planning groups
- Engagement in ward clinical review, if possible.
The candidates we seek would have:
- Bachelor degree or higher in Social Work and eligibility for membership of AASW
- 5 year (minimum) social work experience
- A current National Police Clearance
- Full COVID-19 and influenza vaccination status.
- A demonstrated ability to work both autonomously and within a team
- Excellent communication and organisation skills to work in a fast-paced environment.
