Job description

An exciting opportunity is currently available for an experienced Social Worker (Frailty Virtual) to join the Social Work Team at the Bundaberg Hospital within the Wide Bay Hospital and Health Service. The Wide Bay area is arguably one of the most beautiful regions of Queensland. The Health Service stretches from Agnes Water in the North to Maryborough in the South and provides acute care services to a population of over 225,000 residents including several rural communities.

Eligible Applicants who move to Queensland from an interstate or international jurisdiction to take up permanent or temporary greater than 12 months employment may receive up to $20,000 incentive payments under the Queensland Health Workforce Attraction Scheme ( https://www.careers.health.qld.gov.au/working-for-us/workforce-attraction-incentive-scheme )

Your opportunity:

  • To deliver high level social work clinical services and expertise to patients identified for the rapid access geriatrician-led model of care, as a multidisciplinary team member.
  • This short sub-acute service (up to 14 days) approach aims to reduce prolonged in-hospital patient admissions (extended stays). The position will facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community.

Prior to being made an offer of employment, you will be required to demonstrate immunity to the following Vaccine Preventable Diseases ( https://tinyurl.com/yc6pgyt2 )(VPD): Hepatitis B, Measles, Mumps, Rubella, Varicella (chicken pox) and Pertussis (whooping cough).

Our offer:

You will receive a competitive remuneration package value up to $38,611 p.a., comprising salary between $38.71 - $56.89 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 22.8 hours p.f.).
