Your opportunity:
- To deliver high level social work clinical services and expertise to patients identified for the rapid access geriatrician-led model of care, as a multidisciplinary team member.
- This short sub-acute service (up to 14 days) approach aims to reduce prolonged in-hospital patient admissions (extended stays). The position will facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community.
Prior to being made an offer of employment, you will be required to demonstrate immunity to the following Vaccine Preventable Diseases ( https://tinyurl.com/yc6pgyt2 )(VPD): Hepatitis B, Measles, Mumps, Rubella, Varicella (chicken pox) and Pertussis (whooping cough).
From 9th December 2019 all new employees will be required to be assessed for their risk of Tuberculosis (TB). You will receive a competitive remuneration package value up to $38,611 p.a., comprising salary between $38.71 - $56.89 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 22.8 hours p.f.).
