Seeking a motivated Social Worker to join the Transitional Aged Care Program. TACP provides a 12-week therapy program, case management and support services, with the aim to improve condition, build capability and maximise function to avoid early entry into Residential Aged Care. The SW supports clients by conducting psychosocial assessments, providing counselling, information, and advocacy within a client centred framework, reflecting best practice. The SW will work in collaboration with a large multidisciplinary team, the role is well supported, and professional development encouraged. The position is community based with an SG motor vehicle provided.
This is a temporary part time 32 hours per week leave relief position until 2 February 2025 with the possibility of extension.

What you'll be doing
Provide a high quality social work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above social work service to the Illawarra Transitional Aged Care Service (ITACS). ITACS provides a time-limited, goal orientated service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision making process around long term Care Service supports.
The Social Worker will also maintain social work and case management responsibility for a number of cases until discharge. Duties include early psycho-social assessments, care planning, education advocacy for the needs of clients, families and/ or carers, liaise with community agencies, the client's General Practitioner, counselling and carer support.
The position functions as part of an inter-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care, but are unable to return home without additional care and support.
All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.
Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Please provide proof of vaccination if available.

SELECTION CRITERIA:
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
- Demonstrated ability to work in a health related field and ability to provide a range of social work interventions eg psychosocial assessment, case management, group work, advocacy and education.
- Demonstrated skills in care management of older people with complex needs including an understanding of the Aged Care Act (1997) and Transitional Aged Care Programs.
- Demonstrated ability to work unsupervised, prioritise and manage own caseload using organisational and time management skills.
- Proven ability to work effectively and collaboratively within a multi-disciplinary team.
- Demonstrated ability to communicate clearly and succinctly both verbally and in writing.
- Demonstrated competence in the use of electronic information systems.
- Current NSW Drivers Licence, with the ability to travel in accordance with position responsibilities.
