The program helps older people recover after a hospital stay by providing short-term care for up to 12 weeks. As part of a large multidisciplinary team, the Social Worker role includes completing psycho-social assessments, counselling, working with families and carers, education and future planning. Supporting students and junior staff is an integral part of this position. An SG fleet car is provided, and professional development encouraged.This is a temporary part time 32 hours per week leave relief position until 2 February 2025 with the possibility of extension at same/reduced hours.What you'll be doingProvide specialist Social Work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position provides the above Social Work service to the Illawarra Transitional Aged Care Service (ITACS). ITACS provides a time-limited, goal orientated service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision making process around long term careThe Social Worker will also maintain social work and case management responsibility for a number of cases until discharge. Duties include early psycho-social assessments, care planning, education, advocacy for the needs of clients, families and/ or carers, liaise with community agencies, the client's General Practitioner, counselling and carer support.The position functions as part of an inter-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care, but are unable to return home without additional care and support. Applicants are encouraged to review the following information on Applying for a position within NSW Health at:SELECTION CRITERIA: * Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.

  • Extensive post graduate experience in the provision of Social Work services in a health related setting
  • Demonstrated skills in case management with high level assessment and intervention skills and the ability to manage own caseload through co-ordinated assessment and care management of older people with complex needs.
  • Extensive knowledge of issues related to working with older people and knowledge of Commonwealth Government guidelines on Aged Care residential assessments, the Aged Care Act (1997) and Transitional Aged Care Programs.
  • Demonstrated high level ability to communicate clearly and succinctly both verbally and in writing, and proven ability to work effectively and collaboratively within a multi-disciplinary team. Demonstrated competence in the use of electronic information systems
  • Demonstrated ability to prioritise work and manage stressful situations; ability to work unsupervised and apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature .
  • Demonstrated ability to initiate, lead, complete and share quality improvement activities and service evaluation processes with other staff and demonstrated capacity to provide clinical supervision.
  • Current NSW Drivers Licence, with the ability to travel in accordance with position responsibilities.
