With a focus on person-centred care, you will support the team to provide a high level of quality care to the people we support.
Working with clients in the community, you will be delivering sound social work services including assessments, investigations and treatment planning.
What you’ll be doing
- Provide excellent, evidence based social work interventions to our clients and their families or carers including comprehensive/psychosocial assessment, advocacy, referrals, counselling and care plans
- Evaluate the effectiveness of treatment plans and modify as required
- Participate in care conferences and liaise with other health care professionals as needed
- Maximize client treatment & clinical outcomes
- Educate the general public and community workers in relation to our services and maintain waitlists
- Tertiary qualifications in Social Work and the relevant registration
- Experience in the provision of social work services with adults/elderly favourable
- First Aid & CPR certificate
- Queensland Provisional or above drivers licence
