About Us:A leading provider in social services, is seeking a dedicated and compassionate Social Worker to join our team as a Veterans' Support Specialist. Based in Brisbane, with the flexibility of remote work, this role offers a unique chance to contribute to the well-being of DVA veterans by facilitating their access to crucial allied health services.
Key Responsibilities:
Assessment and Case Management: Conduct thorough assessments of veterans' needs and develop personalized care plans. Manage and coordinate the implementation of these plans, ensuring timely access to allied health services.
Collaboration: Work closely with allied health professionals, community organizations, and government agencies to establish strong networks. Foster collaborative relationships to enhance the quality and scope of services available to veterans.
Advocacy: Act as an advocate for veterans, ensuring their needs are heard and addressed within the healthcare system. Navigate bureaucratic processes to secure necessary services and support.
Remote Support: Utilize technology to provide remote support services to veterans who may face geographical or accessibility challenges. Leverage virtual platforms for assessments, counseling, and coordination of allied health interventions.
Documentation and Reporting: Maintain accurate and up-to-date records of interventions, progress, and outcomes. Generate regular reports for internal and external stakeholders to demonstrate the impact of the program.
Requirements:
Bachelor's or Master's degree in Social Work.
Full registration with the Australian Association of Social Workers (AASW).
Experience in working with veterans, understanding their unique needs and challenges.
Strong knowledge of allied health services and the ability to connect veterans with appropriate resources.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a collaborative team.
Familiarity with remote service delivery and virtual communication platforms.
