The opportunity
Our South Palliative Care Service is currently looking for an experienced Social Worker to join the WA Palliative Care team on a Permanent, 60.8 hours per fortnight contract, this role will be based within our Southern Perth Metro Area, you will be seeing clients within and around Rockingham, Armadale, Yangebup and surrounding areas. The hours are 8.30am - 4.00pm, there is some flexibility in regard to start and finish times.
Key responsibilities
You will work closely with clients, their families and relevant others to take a holistic approach to the social and health needs of clients with an ultimate goal of reducing the number of unplanned admissions to hospitals by successfully integrating the care needs of each client.
About you
To be considered for this role, you must meet the following criteria:
• Tertiary qualification in Social Work and eligible membership of AASW (Australian Association of Social Workers)
• Previous experience working within palliative care and/or health service
• The ability to work within an interdisciplinary team
• Experience in client assessments and care plans
• Excellent communication and interpersonal skills
• High computer literacy
