AddressThe South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West and the Great Southern region of Western Australia. SWAMS delivers a wide range of primary health and community programs and has a strong growth strategy through partnership opportunities and future community development.
This position delivers a quality social work service to Aboriginal people in the Mandurah region that is culturally appropriate, secure and respectful. This position is being offer on a part time basis (.4FTE).
You will be part of a multidisciplinary team providing assessments and coordination of physiotherapy programs including their ongoing evaluation, to improve or maintain levels of physical function for SWAMS clients, particularly those engaged with our Community Services, such as NDIS and aged care programs.
This role will provide culturally secure case management for clients and their family members through the provision of advocacy, follow-up, referral to appropriate services and collaboration with other support agencies. This will include:
