Where you'll be working
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing

The position has a palliative care focus as part of the Community Palliative Care team for Integrated Community Health (ICH) WSLHD but is incorporated within the Hospital in the Home service. There is an expectation that the candidate will assist the HITH Pharmacy team with workload and staff coverage as required.  
To provide quality pharmaceutical services in all areas of the pharmacy department including the safe and accurate dispensing and distribution of medication, home visits, working clinical pharmacy rosters, and providing teaching and education functions to patients and hospital staff.  
This role is responsible to be actively involved in all aspects of pharmaceutical care including:  medication reconciliation, medication order review, clinical pharmacy review, discharge medication processes, patient counselling and provision of patient medilists.
Pharmacist grade 2 are active members of multi-disciplinary teams.

Essential Requirement:

  1. Qualification:  Pharmacy qualifications conferred by Tertiary Education Institutions e.g. Bachelor of Pharmacy
  2. Registration: Full and unrestricted Pharmacist Registration with AHPRA
