About You:
The successful Support Coordinator Client Officer will work creatively and resourcefully with our clients to implement supports in their plan and enable them to explore and reach their goals.
You will be responsible for:
Providing administrative support to Support Coordinators as requested.
Research and creation of mainstream and community-based resources clients can utilise.
Providing ongoing support coordination services to clients in line with their National Disability Insurance Scheme (NDIS) plan.
Using a coaching approach to assist clients in linking in with and accessing activities and services in their community.
Engaging, assessing clients’ needs and coordinating service providers on behalf of clients.
Referring clients to agencies that can provide additional help.
Engaging proactively with clients, stakeholders such as welfare agencies, government bodies and private businesses in professional manner to support the progress clients’ individual goals.
You will be able to demonstrate the following skills:
Communicate effectively through clear and concise language, both written and oral including case notes and report writing
Work independently using initiative and self-direction within scope of practice
Provide a systematic, logical approach to problem solving
Work cooperatively with team members in a productive way
Follow instructions, procedures and utilise resources in an efficient and environmentally conscious way
Provide a high quality and responsive service, actively listening and seeking feedback
Be calm, patient and understanding to participants needs
Build high quality relationships with external stakeholders
Utilise active listening skills in interactions with participants and team members
Communicate openly, honestly, and constructively
Participate in organisational training, meetings, and supervision
Manage time effectively and regularly review and self-reflect personal progress
Work towards and maintain required productivity rates
Essential Criteria:
Relevant qualification with relevant experience or a minimum of 3 years experience through previous appointments
Demonstrated administrative skills using MS word, excel and client management software
Knowledge of the NDIS
Knowledge of statutory requirements
Demonstrated skills and understanding of planning, implementing, and evaluating individual plans
Quality and Safeguarding NDIS Worker Module
Current NDIS Worker Screening Check
Current Working with Children Check
Valid driver’s licence and access to a registered roadworthy and insured vehicle (minimum 3rd party property)
