Premier Fintech is looking for an experienced, exceptional & self-motivated Support Coordinator on a Full-Time basis to join our team at Darwin, reporting to the Team Leader - Specialist Support Coordinator. The right candidate should be able to provide high standards of support and service. This role will be initially offered as a 1-year full-time contract (38hrs/week) with an initial 3 months’ probation. Working hours will be from 8:30am - 5pm (Mon) and 8:30am – 4:30pm (Tue to Fri). This will be a role required to provide individualized support to participants of the National Disability Insurance Scheme (NDIS).
Duties and Responsibilities
Provide support to clients based on their NDIS plan, implementing all supports within the client’s plan including formal, mainstream, and funded supports.
Provide time-limited specialist support coordination to address high-level risk and assist participants in connecting with support and build capacity and resilience.
Providing crisis support to Participants.
Use a strength-based approach when working with clients to establish a Support Plan with the client and other relevant stakeholders.
Build effective relationships with clients and their families while maintaining appropriate and professional boundaries.
Manage client issues and complaints with respect and fairness whilst following policies and procedures.
Ensure regular contact with participants is maintained and recorded.
Prioritize, manage, and administer a caseload/task in a coordinated, efficient, and timely manner.
Ensure that information on relevant community resources is available to clients and appropriate referrals are made and followed up.
Develop and maintain effective working relationships with stakeholders, other Disability Services and Mental Health services, other non-government agencies, and the National Disability Insurance Agency (NDIA)
Complying with all relevant legislation and Codes of conduct
Proven experience working with people with autism, disability, and/or mental illness.
Maintaining an Up-to-date knowledge of the NDIS system and practices, be aware of and ensure all NDIS policies, procedures, and guidelines are adhered to.
Ensure appropriate NDIS Billing for Support Coordination is completed accurately and on time.
Display a commitment to professional development, and participate in and support the team in staff supervision, development, and learning opportunities.
Provide feedback to other service providers.
Ensure client file documentation supports the standards for documentation and meets NDIS guidelines.
Desirable Qualifications and Experience
Experience working in disability services or a similar environment with demonstrated experience in complex disability support.
Relevant tertiary qualification in; Disability, Individual Support, Community Services, or substantial demonstrated experience
Comprehensive knowledge of human rights-based approaches and person-centered principles and approaches, including positive behavior support and active support
Ability to coach, mentor, and develop a team.
Well-developed written, verbal, and listening skills.
Capacity to implement change and identify opportunities for innovation.
Solid financial management capability (billable hours, P&L’s, budgets)
Confidence and capability in using systems and technology and Microsoft Office.
Strong administrative skills: time management, coordination of tasks, efficient work practices Ability to plan and schedule own work independently, monitor progress against work plans and outcomes.
Cultural understanding reflective of the community
Excellent interpersonal skills to engage clients, their families/guardians/carers, advocates, and all other key stakeholders.
