You will be working with NDIS participants to build capacity and implement support to help them achieve their goals outlined in their plans.
The role of a Support Coordinator is to connect participants with an NDIS plan to supports and services in the community. Your role will require that support coordination is completed as agreed, within a timely work schedule and that claims for payment are lodged. This role assumes full responsibility for ensuring that each participant’s plan is implemented, and the participant achieves their goals outlined.
Duties and Responsibilities:
Support Coordination
- Build NDIS participant and family capacity to understand and navigate service system
- Support participants and carers to understand their NDIS Plan, goals and objectives
- Ensure all organisational and administrative reporting and accountabilities are completed accurately and in a timely manner
- Deliver coordination of supports to individuals with an NDIS plan:
- Research, coordinate and manage complex and intersecting supports to suit individuals needs across multiple providers, ensuring client choice & control
- Support participants to build their capacity, resilience and networks with the aim of greater choice and control and independence in managing their personal choices
- Arrange any assessments needed to determine the nature and type of funding required
- Assist participants to prepare for plan reviews by helping them assess whether they achieved their goals and got value for money for their plan, identify solutions to problems experienced in implementing the plan, and to consider new goals
- Assist participants to communicate with the NDIA when required to support their own plan requirements
- Resolve any crisis situations, ensuring that the participant best interest and resilience are supported
