Job Overview:
We're looking for a passionate and dedicated NDIS Support Coordinator to join our team. The role involves assisting NDIS participants in accessing and navigating the services and supports outlined in their NDIS plans. We need someone with excellent communication skills, a deep understanding of the NDIS framework, and a strong commitment to advocating for the rights and needs of people with disabilities.
Key Responsibilities:
Conduct initial assessments to understand the goals, needs, and preferences of NDIS participants.
Collaborate with NDIS participants, their families, and relevant stakeholders to develop personalized support plans.
Help NDIS participants understand their NDIS plans, including budgets, funding allocations, and available supports and services.
Provide information and guidance on available service providers, including helping with the selection and coordination of services.
Advocate on behalf of NDIS participants to ensure their rights and preferences are respected and upheld.
Monitor the implementation of support plans, conducting regular reviews and adjustments as needed.
Maintain accurate and up-to-date records of participant interactions, service agreements, and progress reports.
Stay informed about changes and updates to the NDIS framework and relevant disability services legislation.
Collaborate with internal teams and external stakeholders to improve service delivery and ensure positive outcomes for NDIS participants
Qualification skills:
Previous experience working as an NDIS support co-ordinator.
Thorough understanding of the NDIS framework and principles.
Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with NDIS participants and their families.
Strong organizational and time management skills, with the ability to priorities tasks and meet deadlines.
Empathy, patience, and a genuine commitment to supporting the rights and dignity of people with disabilities.
Ability to work effectively both independently and as part of a team.
Proficiency in using computer software and applications for record-keeping and communication purposes.
