Role Description: As a Support Coordinator, you will play a pivotal role in assisting NDIS participants in achieving their goals and enhancing their quality of life. You will work closely with individuals, promoting their choices, and providing them with the support they need to make informed decisions about their NDIS support plans. You will be an advocate for their needs and work diligently to ensure they have access to the best services and resources available.
Responsibilities:
- Provide support to NDIS funded participants, assisting them in navigating the NDIS system and advocating for their needs.
- Work collaboratively with participants to identify their personal goals and develop individualized support plans.
- Monitor and assess progress towards goals, regularly updating reports, and sharing updates with supervisors and case managers.
- Promote the mental and physical wellbeing of clients by supporting them in household tasks, outings, hobbies, and learning activities.
- Identify additional learning requirements and facilitate access to suitable learning and skill-building opportunities.
- Collaborate with the RAW (Reading and Writing) adult learning support program, engaging participants and ensuring they have the necessary support to succeed.
Requirements:
- A strong passion for helping individuals achieve their independence with disabilities achieve their goals and live fulfilling lives.
- Excellent communication and interpersonal skills to build strong relationships with participants and their families.
- Ability to work collaboratively with supervisors, case managers, and other stakeholders.
- Strong organizational and time-management skills to maintain accurate records and meet deadlines.
- Empathy, patience, and understanding in dealing with diverse needs and challenges.
Desirable Qualifications:
- Qualifications or experience not necessary, a good attitude and willingness to learn is necessary.
- Familiarity with the NDIS framework and the disability services sector.
