We are seeking a Registered Nurse who is looking for work/life balance.
Reporting to the Service Manager, the Support Partner role is responsible for ensuring quality care and supports are delivered in a customer-centred way to enable more older Australians to remain living at home safely.
You will be responsible for Telehealth assessment and care planning and ensuring that customers have the tools and knowledge to independently manage their care and support their goals.
A bit about the day to day:
- You will be responsible for working with the customer and their families to develop and review Care Plans and budgets that meet their individual goals.
- Ensure customers understand what they can and can't spend their Home Care Package funds on and have the tools to manage their supports independently within their budget.
- Regular customer reviews to ensure the customers are utilising their funding in the optimal manner and provide any guidance to support the successful ongoing independent management of their funding.
- You will have a strong focus on compliance and documentation, understanding the regulatory responsibilities and ensuring policies and processes are followed to enable the customer to safely and confidently mange thrown supports based on their goals and budgets.
A bit about you:
- 12 months Care Manager experience, ideally with a Self Managed Home Care provider or with an openness to Telehealth based Care Management and supporting customers to manage their own supports independently
- Excellent administrative and computer skills including being able to type while conducting Telehealth client assessments.
