Moorditj Koort Aboriginal Corporation (MKAC) is an Aboriginal community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia.

Position summary:
The Transition Care Program is seeking an experienced, innovative and flexible individual to work as a member of the TCP Team.
The program aims to assist Elders to reach their maximum functioning capacity and to help determine their long-term care arrangements.
A successful applicant will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues. They will also need to be highly motivated, enthusiastic and be able to work both independently and as part of a team.
The Support Worker within our Transitional Care Program (TCP) is a crucial role in improving clients' quality of life and promoting their independence. They prioritize the safety and welfare of clients, fostering open communication with both clients and their families, as well as healthcare professionals.
The successful candidate must have the following essential certifications.
- Qualification in Individual Support or similar
- WA Valid Driver’s license
- Police Clearance no more than 3 months old (Mandatory)
- Vulnerable Peoples check
- First Aid or willing to obtain.
- COVID-19 Vaccination Certificate
Skills, Abilities and Knowledge
- As directed, work actively as part of the aged care and community services team with clients in line with Client Care Plans
- Develop and implement programs to help develop client skills and abilities to be able to make decisions affecting their lives, live as independently as possible and to help them participate in the community.
- Encouraging clients to establish personal, community, and social relationships for their well-being.
- Advocating for clients to ensure their care and cultural needs are met, and addressing any issues that may arise.
- Actively participating in team meetings and reviews to coordinate care and ensure holistic support.
- Encourage clients to develop personal, community and social relationships.
- Maintain Regular communication with patients to understand their needs and provide appropriate support.
- Ensuring that all patient-related matters remain confidential.
- Provide necessary reports and Data collection as required.
- Building and maintaining relationships with key stakeholders, including service providers, to facilitate referrals and enhance support pathways.
