About the Role
Provide support services and mentoring to NDIS clients and/or young people living in out-of-home care, assisting them in achieving their goals to reach their full potential. You will be required to work as part of a team to encourage clients to complete activities relating to daily living, community engagement, basic home maintenance and development of life skills.

Skills & Experience
• Applicants with a Diploma or Certificate IV in Community Services or a similar level of qualifications in relevant fields. OR at least 2 years’ experience working as a youth or NDIS worker would be considered. OR if you are willing to undertake this qualification within 3 months of employment.
• Current Positive Blue Card and Yellow Card Notice- or eligible to obtain.
• Current First Aid and CPR certificate – or willing to obtain.
• Current Driver’s License with fully comprehensive personal vehicle insurance (we have a fleet of vehicles; however, you may need to use your personal vehicle at times. Milage is compensated at sector rates).
• Ability to build & maintain rapport with young people or clients with a disability to attend to their daily care needs.
• Ability to deliver appropriate person-centred support services for young people and clients with disabilities.
• Demonstrated experience completing detailed case notes and Incident reports.
• Ability to work collaboratively and effectively with team members, young people, clients with a disability and other relevant stakeholders to always ensure consistency of care.
• Knowledge of the relevant legislation or the ability to gain knowledge (NDIS client charter & Child Protection, Statement of Standards of Care).
• Highly developed interpersonal skills, with the ability to relate to all management, staff, and clients.
• Understanding or ability to gain knowledge of managing challenging and complex behaviours.
• Responsibility for cleaning tasks during shifts.
• A clear understanding of strict confidentiality and privacy policy requirements
• Ability to document and manage Conflict of Interest issues as required.
Key Responsibilities
• Effective communication during shift handovers.
• Provide consistent care and emotional support to all clients.
• Treat all clients with respect and regard given for the individual human rights & Standards of Care for Child Safety and or NDIS Standards of Care.
• Assist clients with daily living activities, including personal care, meal preparation, and household tasks.
• Fulfilment of all assigned tasks by the end of each shift.
• Maintain accurate and up-to-date records of client progress and provide reports as required.
• Work collaboratively with other healthcare professionals, including doctors, psychologists, and social workers.
• Maintaining confidentiality and professionalism in all job aspects.
• Other duties as assigned by the CEO or Operations Manager.
