What you'll be doing
The Clinical Coordinator (CC) will be managing new referrals to the SNF Service and intake, undertaking initial client (family) engagement and clinical assessments, and providing clinical consultation for team members, leading case reviews and may relieve clinical staff during short periods of leave.
The CC will lead, manage, and inspire their team to provide a high standard of safe, evidence-based, effective, and person-centred care for infants and their caregivers. The position manages and supports all SNF Child and Family Health nurses, and the SNF Social Workers.
People of Aboriginal and/or Torres Strait Islander background encouraged to apply.Candidates will need to meet the following criteria:
- Registered Nurse-Division 1 with Australian Health Practitioner Regulation Agency (APHRA) and relevant management tertiary qualification or equivalent work experience, or a combination of study and work experience; and demonstrated commitment to professional development.
- Extensive post registration experience in Child & Family Health Nursing (CFHN).
- Experience working with vulnerable families and a demonstrated understanding of early intervention, including the theoretical basis that underpins sustained home visiting, prevention, and community development.
- Experience in managing a team and knowledge in strategic and operational service planning and quality improvement processes.
