- Provide direct client care and care coordination ensuring that services are provided in accordance with the Rehabilitation at Home model of care
- Manage the program’s intake function including cohort allocation and oversight
- Work collaboratively with a wide range of stakeholders including SVVH colleagues and external service providers
- Contribute to the development and delivery of virtual care solutions nationally
- Drive improvement initiatives and service development activities to support the growth of the program nationally
- Relevant clinical experience in acute and/or community setting
- Current nursing registration with AHPRA, as well as a current Victorian motor vehicle license are essential
