We have an exciting opportunity for a Systems Integration Coordinator to join our future focused and rewarding organisation. This role reports to the Director - Primary Care and is a key member of the Primary Care Team.
Darling Downs and West Moreton PHN is a not for profit organisation that works at a regional level to achieve improved whole of system health care.
Our key objectives are to:
- increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
- improve coordination of care to ensure patients receive the right care, in the right place, at the right time.
Our PHN achieves this by working with local health care stakeholders and community to:
- fund programs and services based on local health needs;
- support primary health care professionals, like GPs, general practice staff and allied health professionals, to improve the care they provide to their patients; and
- support the development of a local sustainable health care workforce.
About the role:
As the Systems Integration Coordinator you will be tasked with a wide variety of responsibilities that assist with the continual maturity of the organisation. The current areas of focus for the Systems Integration Coordinator is to work closely with General Practice stakeholders and the in-house GP Workforce Planning and Prioritisation (WPP) data analyst to deliver the Darling Downs and West Moreton PHN’s contribution to the GP WPP program. This work occurs in the context of the PHN’s GP Workforce Collective Impact Action Plan. The role will also develop and execute system integration strategies across the local health system to reduce potential avoidable hospitalisations and emergency department presentations. This includes supporting the Medicare Urgent Care Clinics in the region and other hospital avoidance initiatives.
Key purpose of the role:
- Develop and execute system integration strategies across the health sector.
- Work across the organisation to streamline projects and priorities.
- Build successful relationships and partnerships with key stakeholders both internally and externally.
- Participate in regional planning, integration and implementation of the PHN’s Healthy Primary Care Plan.
Required Attributes:
- Establish and maintain productive working relationships with General Practices, GPs, practice managers, and ACCHOs to inform WPP activities.
- Develop the workforce needs reports and training capacity reports, including evidence-based recommendations, as part of the WPP program for the PHN region. Plan and coordinate consultations and surveys with General Practice teams, Registrars and GPs in the PHN Region to seek feedback on local WPP recommendations.
- Work collaboratively with GP Advisors, liaise with existing Registrars in the catchment to determine suitability of the training placements, opportunities for gaining advanced skills, and other issues that may affect trainee wellbeing.
- Work with system partners including, Hospital and Health Services to design and deliver initiatives to reduce potential avoidable hospitalisations and emergency department presentations.
- Oversee and coordinate multiple projects and initiatives utilising an evidence-based project management methodology.
- Coordinate and manage the design and implementation of complex monitoring and evaluation frameworks, incorporating a community approach throughout.
- Oversee data and shared measurement approach across this initiative including liaison with key stakeholders.
- Develop business cases, project plans and reports in alignment with project requirements.
Selection Criteria:
- Relevant experience in Health or human services, public health or health service planning.
- Experience in effective stakeholder engagement, partnership brokerage and relationship development with the mental health community including consumers, carers, advocates, providers, policy makers and others.
- Experience in a human services environment, particularly with sector development models, resources, clinical governance, processes and tools to support and enhance system reform.
- Experience in program development and project management to affect systems change including a good understanding of data analysis, project evaluation and reporting.
- Highly developed interpersonal, communication and negotiation skills and the capacity to build and maintain relationships with DDWMPHN stakeholders.
